Use this Child-Pugh Score calculator to determine the severity and estimate mortality in cirrhosis. It also informs you about the Child-Pugh class and survival probability.

What Is the Child-Pugh Score?

The Child–Pugh score (Child-Turcotte-Pugh or CTP score) is used to evaluate the severity of cirrhosis and assess the need for liver transplantation. It considers the following clinical parameters:

  • Bilirubin
  • Albumin
  • Ascites
  • Encephalopathy
  • Nutritional status

The resulting score helps classify patients into three risk classes (A, B, or C) based on disease severity.

Child-Pugh Scoring Chart:

Parameter 1 point 2 points 3 points
Ascites Absent Slight Moderate
Bilirubin (µmol/L) <34 (<2 mg/dL) 34–50 (2–3 mg/dL) >50 (>3 mg/dL)
Albumin (g/L) >3.5 2.8–3.5 <2.8
Prothrombin time (seconds over control) or INR <4 or <1.7 4–6 or 1.7–2.3 >6 or >2.3
Encephalopathy None Grade 1–2 Grade 3–4

What Is Cirrhosis of the Liver?

Cirrhosis is the stage of liver scarring (fibrosis) in which healthy liver tissue is replaced by scar tissue. This prevents the liver from functioning normally and may lead to liver failure.

Cirrhosis of the Liver

Cirrhosis can result from chronic liver diseases, including:

  • Hepatitis B, C, or D
  • Autoimmune hepatitis
  • Cystic fibrosis
  • Excessive alcohol consumption
  • Other liver conditions

Hepatic Encephalopathy Staging – West Haven Criteria

The stages of encephalopathy are important parameters in assessing the Child-Pugh score.

Hepatic Encephalopathy – Stages (West Haven Criteria)

The five stages of hepatic encephalopathy are:

Grade Clinical Findings
Unimpaired No symptoms present
Normal psychometric or neuropsychological tests
Grade 1 Mild symptoms: shortened attention span, sleep pattern changes (hypersomnia or insomnia)
Abnormal psychometric or neuropsychological tests
Grade 2 Moderate symptoms: disorientation to time, subtle personality changes
Grade 3 Severe symptoms: inability to perform basic tasks, confusion, pronounced personality changes
Grade 4 Coma

Today, the MELD score is mostly used for liver transplantation prioritization, but the Child-Pugh score remains a valuable system for assessing liver disease severity.

How to Interpret the Child-Pugh Score

The Child-Pugh score classifies patients into three categories based on the severity of liver disease and predicts survival probability:

Class A – Least severe liver disease Class B – Moderately severe liver disease Class C – Most severe derangement
Points 5–6 7–9 10–15
1-year survival 100% 81% 45%
2-year survival 85% 57% 35%

Child-Pugh Score and Surgery Guidance

  • Class A: Patients are in a safe condition for elective surgery.
  • Class B: Patients are at higher risk but can still undergo surgery.
  • Class C: Patients are not in a safe condition for elective surgery.

For a quick assessment of cirrhosis severity, use the Child-Pugh score calculator. It calculates the Child-Pugh score, estimates survival probability, and indicates disease severity.

FAQ's

What Are The Signs Your Liver Is Shutting Down?

Some common signs of cirrhosis and declining liver function include:

  • Weakness or fatigue
  • Easily bleeding or bruising
  • Swelling in the legs, feet, or ankles
  • Itching on different areas of the body

What Is The Longest You Can Live With Cirrhosis?

Survival depends on the stage of cirrhosis:

  • Compensated cirrhosis: may survive more than 12 years
  • Decompensated cirrhosis: survival is typically around 2 years
